Registration Forms

Forms must be submitted by August 7th. Please note that students will not be permitted to participate in preseason athletics, regular season sports, or attend classes until all required health forms have been received.

(2) Medical Forms

Healthier students are better learners. We want to support your child’s health to make your child more successful inside and outside of school. To help manage your child’s health needs in school, we require you and your child’s health care provider to complete certain forms.

The following health documents are required to be submitted yearly by August 7th. Students with health forms not submitted prior to the first day of school will not be permitted to attend school, nor participate in field trips or athletic activities.

Notes:
Immunizations
In accordance with Rhode Island State laws, all students are required to provide proof of up-to-date immunizations or annual certificate of exemption before attending school. Immunization requirements and exemption forms can be found HERE. Documentation can be emailed, faxed, or dropped off to the health office.

Medications
All medications, prescription or over-the counter, require a yearly medication authorization form. No medication will be administered without specific written instructions from the students parent and/or physician.
 
Students may not keep medications (prescription or over the counter) in their possession during the school day. All medications with written authorization and instructions for administration, should be given to the school nurse. All medications, prescription and non prescription, should be in the original, properly labeled containers. 
HOURS AND LOCATION
The Providence Country Day Health Office is located on the second floor of the Metcalf building. Nurse Costa is on campus Monday-Friday 7:30am-3:30pm.

(3) Athletics Forms

The Athletic Department welcomes you to PCD! We are confident that your participation in the athletic program will be enjoyable and the experience gained will contribute to your growth and development at The Providence Country Day School. The following links will direct you to important preseason information and the athletic participation guidelines. Your fall sports selection must be made as part of the online Sports Selection Form, no later than August 7th.

The PCD Athletic Department represents the School’s largest extra-curricular activity. Our goal is not only to allow athletes to reach their maximum potential in a particular sport, but also to help our student-athletes gain a life-long appreciation for physical fitness. Our intention is that students learn life lessons, such as the value of teamwork and hard work, and also benefit from the uniquely human experience of sports simply for enjoyment and fun. Toward this end, we require each student to fulfill a minimum athletic participation requirement

Please remember that you must have the following forms submitted in order to participate in preseason practices:
  • RI School Physical Exam
  • Student Health History
  • Emergency Authorization
  • RIIL Assumption of Risk 
  • Concussion Information Forms
